A Personal Welcome Message from Don Rosenberg

This is Don Rosenberg, President of Shorehaven Behavioral Health, where we improve lives through effective mental health services and a culture of lifelong staff learning and training, where all persons deserve the same high quality of services.

I want to take a few minutes to give you some key points to go along with the welcome message on our home page.

We have been treating mental health and substance abuse problems for over 40 years.

About 50% of our clients are children or adolescents, and their families.

We are one of the largest providers in our area of psychotherapy for people on state insurance, also known as Medicaid or Badgercare. We also work with people who have commercial insurance through their work.

We develop a broad, rich skill based in our therapists. So, we can help people with almost all mental health problems.  If we lack sufficient expertise, we work on recommending the resources you need.

We are a respected training clinic, meaning over a dozen universities send their students to us for more advanced training.

When a person has both substance abuse problems and mental health problems, we treat you not one problem at a time, but in integrated care where we work on the problems together for a more effective outcome.

We also have an in-home program for children and adolescents who have more serious emotional problems.  That program is family-based – we work with everyone in the family.

A word about depression, anxiety, and other problems.  Most problems have evolved from our life experiences, the thoughts we developed from those experiences, our emotional reactions, our long-standing beliefs about ourselves and our human environment, and how all those factors affect our relationships.

Let me illustrate.  Let’s say a person reports being depressed since losing a job, which triggers feelings of shame and sadness, which accompanies feelings of inadequacy, which elicits old memories of past times when the person did not do well in school or when a parent scolded the person with harsh words, and then he or she has thoughts of being embarrassed, and then withdrawal from others.  In that cascade of reactions, depression is almost inevitable.

Our job as therapists is to help you unravel these complex reactions. By understanding them, you have the power to make changes in your thought processes, to bounce back, to overcome depressed mood, and to flourish.

Similarly, we help to sort out the patterns behind anxiety, post-traumatic stress, behavior problems, drinking problems, the host of mental health and substance use problems.

Using the latest research and evidence-informed methods, we can help about half of callers in 3-4 months and 80% of callers within 6 months, about 25 sessions. Some problems are multiple, long-standing, and complex and those require long-term help.  In our section on Preparing for Therapy, listen to the 5-7 minute audio recordings that explain all about therapy.  In our Resources section, read our paper about real Recovery-Oriented Thinking and another about being the best therapy client you can be. Insurance and Sliding Fee Program:

We accept Medicare, Medicaid, and most employer-based commercial insurance plans. Our offices serve all clients regardless of ability to pay our full fee. As members of the National Health Service Corp, we offer a discounted/sliding fee scale based upon family size and income.

Treatment Works

Whether it’s called counseling, therapy, or psychotherapy, we understand that deciding to call for help may be intimidating for some, a relief for others, and a message of hope – the hope of healing, recovery, and growth.

You may be uncertain about how psychotherapy works. First, let's assure you that psychotherapy is based upon research evidence. Psychotherapy works.  Up to 80% to 90% of people experiencing emotional or behavioral problems improve in therapy.  Most clients experience moderate to very significant improvement. That is a remarkable record of success.  So, make it a priority. 

When you put commit to making therapy a priority, you should improve as well.  If the problem is with the behavior of a child, by working together with us as a family, your can help your child should.

The emotional problems we can help you with include, among many others:

1. Adult Personal Problems

How to Become a Client

Get Your Therapy Off to a Great Start!

Here are a few pointers to make sure we can help you as quickly as we can:

Be prepared for the first meeting by thinking about these questions.

  • What symptoms and behaviors concern you?
  • What outcomes will let you know you have made progress in therapy?
  • What recent events in your life have been difficult to handle?
  • What past events in your life are still upsetting or painful for you?

Bring along the names, addresses, and phone numbers of any professionals who may be involved with your care:

  • your primary care doctor
  • your psychiatrist if you have one
  • any past therapist(s) or psychiatrists(s)
  • ALL medications you take for any reason – including names and dosages, including vitamins, herbals, and supplements
  • your lawyer, if you are involved in any legal proceedings
  • your case manager, if you have one

Any documents which might be relevant and helpful, such as:

  • insurance card
  • previous psychological assessments
  • relevant court orders and custody decrees
  • records from any psychiatric hospitalizations and past psychotherapy
  • medical evaluations
  • children’s recent report cards

One Call and Recovery Begins

  • Our Referral and Scheduling Department maintains a list of appointment openings for new clients.
  • We work to get people into first appointments quickly. Unlike many clinics that maintain waiting lists, at Shorehaven, we know that rapid access to help leads to faster and better outcomes.
  • When you call (414) 540-2170 during working hours, 9:00 a.m. — 5:00 p.m., Monday through Friday, an intake specialist will take down the information we need to help you.
  • If you call after hours, just leave a message on (414) 540-2170, and we'll call you back the next work day.  Expect your first call to take 10-15 minutes.
  • You can also print out the forms located below or to the right, fill them out, and fax them to (414) 540-2171.  Then, we'll call you to set up an appointment.

The availability of new appointments depends upon several factors, including the problem you are having, when you may need to schedule, your location, and your insurance. The majority of the time, we have an opening that matches your needs. Occasionally, there may be a short wait for just the right match.

What Will Happen At Shorehaven

Our aim is to make you feel you have made the right choice, the best choice–to get help NOW and from a highly respected clinic.

The work we do is called therapy or psychotherapy. You can read about therapy in our flyer What Is Psychotherapy?  There are many forms of therapy, but they all come down to techniques of psychology which are designed to help bring about change. We use treatments backed by scientific research.

The process begins with a session devoted to evaluating the causes and effects of the problems you want to change. Much like a doctor, we'll get detailed information about the problems, your past history, and your goals. After that we'll give you an explanation of the problem and some recommendations. Then we will schedule times for the therapy to begin. It's so very important to come to every scheduled meeting.

Begin Your Recovery At Shorehaven and See It Through

When you start therapy, understand that problems that only began recently often improve in just weeks to a few months. That is a lot like physical therapy for rehab after surgery or an accident.  Problems that are more long-standing and complex may take months up to a year or more.  That is a lot like treatment for any chronic medical problem.  Research shows only about 20% of callers need 12-24 months of therapy and about 10% need long-term therapeutic support.

Please Complete Forms

We will need some information in order to help you, your child, or your loved one. We'll need information on a CONSENT TO TREATMENT, TELEHEALTH, & PAYMENT PLAN form. We’ll also need a HISTORY so we know enough about the problem to get therapy off to a good start. 

You’ll be pleased to know that state and federal laws govern how Psychotherapy is delivered. For instance, we observe a high level of confidentiality. This is explained in the brochure, Helping You Find Your Strength and Serenity.

We may need an Authorization for Disclosure of Confidential Information. The latter is used to ask your permission to contact the persons you list on the forms, be they your physicians, insurance company, case manager, or attorney.

Our consent form will additionally be explained in further detail at your first session with the therapist so you can make an informed choice about your therapy.

These are the forms every therapist in Wisconsin needs in order to begin counseling. We will have you fill out copies of these forms in the waiting room or you can print them out and complete them at home and bring them with you. If your first session is offered via telehealth, you will have the option to sign these forms digitally before the session.

In addition to the CONSENT FOR TREATMENT, TELEHEALTH, AND PAYMENT PLAN, we’ll also need a copy of both sides your insurance card. You can also copy both sides of your insurance card and fax to 414-540-2171, or just bring it all to the first appointment.

If you have state insurance, also known as Title 19, Medicaid, or Badgercare, we just need the client’s MA# or a copy of the Forward Card. Our secure fax is: (414) 540-2171.

Naturally, this is an important moment.  When people are ready to be helped, we want to get that help for them rapidly.

Read How to Make a Good Referral.  Let’s summarize some key points:

  • Complement the person and encourage them.
  • Avoid criticisms, such as, “It’s about time.” 
  • Make it easy by calling us or just giving the person our phone number.  Perhaps, offer to drive or accompany the person.
  • As the time for the appointment draws near, people may think about reasons to avoid it, so let them know they may have mixed feelings about it, but go anyway, no matter what.
  • Affirm how positively you feel when you relative or friend has followed through.

Psychotherapists and Psychiatrists

If you are a therapist referring to us for DBT, AODA services, In-Home, or Outpatient, again, thank you.  It would be helpful to know what information you may have about the client and the reason for your referral.  You may have some idea about diagnosis, problems, goals, and family.  Any information you can provide or tell the client to inform us about will be helpful.  If you have files on the client, we can send you a Release of Information form signed by the client.  The client can call us or you can provide a phone number and we will make the call.


Discharge Planners or Discharge Social Workers

Thank you for your referral. We are aware of HEDIS standards and will make sure you have an appointment set so you can have it in your records. 

1) We ask for an important element in your handover of the client, namely, please get a Release of Information and send us the Discharge Summary as quickly as possible.  You can email it to Referrals@Shorehavenbhi.com or fax to 414-540-2171.  

2) The other crucial element in your handover is letting the client know everything about the appointment – time, date, place. We will need the client’s email address and telephone.  We will contact the client.  Then we can send the client a welcome message.

Please direct your referral to the forms below or on the right. If the client is a child under 18 who is not living with his or her parent or guardian, the parent or guardian must sign the forms. You can have them completed and fax them to us as well.


Case Managers or Case Coordinators

Thank you for including us in the treatment team.  If you represent an insurance company or payer, please include a prior authorization, if one is needed.  You have a lot of information on the client that will be an important part of our assessment and treatment, so, consider any documents or data a therapist should have in order best to understand the client and the treatment goals.  With a Release, please send us any assessments and plans that might help. This could be a Psychiatric or Psychological Assessment, a Protective Service Intake Report, a Case Plan, a Family Reunification Plan, or any other documents that will be essential to quality services. 

If the client is a child who does not live with his or her guardian, our Consent to Treatment form and Authorizations for Disclosure of Confidential Information must be signed by the legal guardian.  We will need such Releases for your agency, another for the foster parent, another for the child’s school, and another for the child’s doctor – 4 releases in all.


Medical Offices

If you are a Physician, Nurse Practitioner, PA, or a staff member, we thank you for entrusting us with the care of your patient.  If you call us directly, we would benefit from knowing the problem for which you are calling.  With the client’s written permission, we would also benefit from a list of diagnoses and medications that you have on file.  We will in turn let you know when the client has come.


Attorneys

If you are an Attorney or Paralegal or staff in a law office, thank you for your referral.  We would like to know the reason for your involvement with the client.  While we do not do custody evaluations, we work with many couples who may be in conflict, separated, or divorced.  We will benefit from having some idea of what you are hoping for from therapy for your client.  If there is a pending deadline or court date or a date when a form or assessment is due, please make that clear to us.

Preparation for Psychotherapy is an important part of achieving the best outcomes.  We have so much to tell you about preparing, we recorded some short audio, about 6 to 8 minutes each, to answer your questions.

Tele-therapy

Sometimes called online therapy, telehealth, or teletherapy, it is a form of therapy that lets you connect with a mental health care provider online rather than in person. These virtual visits can take place over the internet via a computer, tablet, or smart phone. Shorehaven uses Avaya Spaces, a HIPPA-compliant platform for telehealth video sessions that can be easily downloaded onto your tablet or phone or accessed on the computer via your preferred web browser. 

The coronavirus pandemic increased the need for help with mental health challenges. Shorehaven's staff accomplished an amazing and successful shift to tele-health and tele-therapy during the Covid-19 Pandemic, and has continued to offer telehealth alongside in-person therapy. An advantage to tele-health is that we are able to increase our hours of therapy, helping more Wisconsin residents meet their therapeutic needs  -- wherever you may be in Wisconsin.

Sliding Fee Discount Program

Our commitment extends to everyone, even if you don’t have access to health insurance, or if you don’t think you can afford services. We offer the option to apply for our Sliding Fee Discount Program. This program helps people who have no health insurance, or those who have only limited health insurance, so that they can get the mental health or substance use recovery care that they need.

To apply for the Sliding Fee Program, talk with one of our Referral Department specialists who will explain the program and the information we need. Approval for the program is set by a federal standard that is based on family size and combined household income in accordance with the current year Federal Poverty Guidelines.

You can only be evaluated for participation to receive a discount under the Sliding Fee Scale if you provide the required information.

If you are enrolled in other assistance programs, (such as Badgercare), Federal rules do not permit you to use the sliding fee schedule in lieu of using your insurance.

Shorehaven provides services regardless of ability to pay our full fee.

History & Philosophy

Take a moment to learn who we are and what we strive to do well.

Don Rosenberg, President of Shorehaven, has been a practicing Psychologist, MFT, and Substance Abuse Counselor, beginning in 1973.  He became the President of Shorehaven in 1996. Lynn Godec, Executive Director of Shorehaven, joined in 1998.  Together, they merged three practices into Shorehaven Behavioral Health, Inc., in its present Brown Dear headquarters in 2004.

Over the years, we added locations in Greenfield and Racine (Mt Pleasant).  As early as 1998, we began training new graduate students and recent graduates. We grew into the largest training clinic in the area. A training site must provide state-of-the-art care and that is our constant goal.

Starting in 1997, we began to work with substance abuse and addiction problems.  We have worked with couples, families, and children since the beginning.  We have been the largest center for the therapy of children in the area.


During our two decades of serving the residents of Wisconsin, our philosophy is to treat all patients as equal partners in their healing.

  • EASY TO ACCESS -- We provide ACCESSIBLE Treatment Services. We try to assist callers to make outpatient appointments quickly and easily. We try to minimize wait times for help – usually to just 3-10 days.  Unlike the usual long waits for care in this area, we structured our systems in order to deliver care quickly.
  • CONVENTIENCE-- We make services convenient: multiple locations, evening hours, with in-home services in several counties.
  • DIVERSITY -- All persons in need of our help are welcome. We have always avoided discrimination and promoted equality and diversity in our staff and clientele.
  • OPEN TO ALL -- In addition to consumers insured through employers, we also welcome all Badgercare/Medicaid and Medicare consumers in need of mental health services.  Unlike many clinics in the region that limit access for those on State insurance, we welcome all.
  • UP-TO-DATE -- Our treatment services honor the latest research on state-of-the-art ways to help.
  • CHILD & ADOLESCENT CARE -- We strive to develop the best and largest child and adolescent treatment center in our region.
  • FULL SERVICE -- We aim to offer a full range of services - the care that's needed when you need it:
    • Individual therapy
    • Child therapy and adolescent therapy
    • Couple, marriage, and family therapy
    • EMDR: eye movement desensitization and reprocessing
    • In-home therapy
    • Help for virtually all mental health problems
