THE INTENSIVE IN-HOME PROGRAM and IN-HOME REFERRAL CHECKLIST


First we will describe the program.
Below is a checklist to determine if the child qualifies for in-home treatment.

SED Intensive In-Home Program

The Program

We provide intensive in-home child and family therapy to as many as 100 families at a time, delivered by experienced teams of mental health professionals. The Program is:.

Intensive: 4-8 hours per week, on average, in 2-5 weekly visits to the family

In-Home: Most treatment services are provided in the home, helping to ensure family participation. School consultation is also part of the program.

Child and Family Therapy: The child or adolescent receives psychotherapy and behavior therapy while the family receives treatment for interactional, parenting, ecosystem, and inter-generational problems.

Experienced Teams: In-Home at Shorehaven is provided by licensed, certified, trained clinicians rather than only para-professionals. Our staff averages 10 or more years of experience, whereas most in-home services use less experienced providers.

The Goals

  • In-Home focuses on crisis management and lasting change rather than merely on stabilization, targeting changes across a wide range of problems, including developmental, behavioral, psychiatric, academic, and family problems.

  • In-Home focuses on prevention of out-of-home placement, such as residential, group home, or inpatient, by intervening in troubled family systems.

  • In-Home brings change into the child°Øs ecosystem of family, school, medical, and community systems for more lasting changes. Treatment is individualized, comprehensive, long-term, and multi-systems.

The Treatments

  1. A team of two, sometimes three, mental health professionals allocates from ten to over thiry treatment hours to each family per month, as needed.

  2. We triage the child's needs and develop a comprehensive treatment plan. Services are provided for up 15 months. The plan may involve simultaneous day treatment, outpatient, or psychiatric services, and long-term outpatient follow-up care. The plan always involves coordination with other services, such as psychiatry, foster care case management, and school services.

  3. We strive for

    • Rapid accessibility. Our new Crisis Service for troubled foster home placements aims to solve problems which may cause a foster placement to fail so the child does not have to change homes.

    • Coverage of the in-home treatment needs of children across the entire region including Milwaukee, Jefferson, Waukesha, Ozaukee, Washington, Kenosha, Racine, Walworth, and Rock counties,

    • Services can be provided in Spanish in Milwaukee.

    • In-Home is a long term program, up to 60 days in Safety Services, from 3 to 15 months for on-going or Medicaid SED in-home services.

  4. Psychotherapy • Behavior Management • Behavior Therapy • Social Skills Training • Systemic Family Therapy • Anger Management • Specialized Treatment for anxiety, depression, severe emotional disorders, AD/HD, disruptive behavior and behavior disorders. • Parenting skills

The Advantages

This program has important advantages over Residential, Inpatient, Outpatient, and Mentoring, which should make In-Home a first choice for many families. We team effectively with local Child Day Treatment, Child Psychiatric Inpatient, Child Welfare, and school programs. In-Home helps ensure participation of the child/adolescent and the family since services are brought to the home.

Does the Child Qualify for the Intensive
IN-HOME SED [Seriously Emotionally Disturbed] Program?

CRITERIA

We have immediate openings in our in-home program for youth who meet these standards.

A. Necessary Demographic Factors

  • Youth Aged 3 to 20 and Funded by Medicaid (Title XIX), Wraparound, Safety Services, or On-going.

  • If child has a Medicaid HMO, dis-enrollment (exemption) or for HMO approval is possible.

B. Three Necessary Emotional-Behavioral Factors

  • Must have a Serious Emotional Disturbance, that is, the youth meets the criteria for a DSM-IV Diagnosis, such as Post-Traumatic Stress Disorder, Panic Disorder, Pervasive Developmental Disorder, Separation Anxiety, Depression, Bipolar, AD/HD, Oppositional Defiant, Adjustment Disorder, Conduct Disorder, Substance Abuse, Major Depression, Reactive Attachment, etc.

  • Must have received some previous services from 2 or more of these: special education, inpatient psychiatric hospital, outpatient mental health clinic, mental health day treatment, probation, social services, child protective services, juvenile justice, residential treatment, etc.

Must have ONE of the following:

  • Two areas of Psychosocial Impairment in the domains of family relationships, peer relations, school functioning, self care management, or impulse control.

  • OR either suicidal ideation/impulses, frequent aggressive behavior, or psychotic episode.

C. Two Necessary Commitment and Motivational Factors

  • Family acceptance of intense intervention in the home by 1 or 2 therapists for 2 to 8 hours per week. Family is able to work towards goals. [May be a foster family.] Family capacity to follow-through in order to work with our team over a 3 to12 month period, and to be present reliably for meetings, to assist getting Healthcheck and psychiatric assessments.

  • Parent or legal guardian available to sign a Consent for Treatment form and an Authorization for Disclosure of Information form when we accept the child into the program.
